Lafayette, LA – Anesthesiology & Pain Consultants, LLC (“A&PC”) experienced a data incident that may have affected a limited number of individuals’ information. On October 21, 2025, A&PC identified that on October 19, 2025 an employee or employees copied certain information from its network to an external drive(s). A&PC immediately began an investigation to determine the specific type and extent of the information copied. The investigation determined that certain patient information was included in the copied files. Therefore, A&PC reviewed the files in order to notify potentially impacted individuals. On December 17, 2025, A&PC completed the review. The type of information identified varied by individual, but may have included name and one or more of the following: address, telephone number, date of birth, Social Security Number, Patient Identification Number, Medical Record Number, and medications.

Although the external drives have been recovered, as part of its commitment to protecting patient information, A&PC is also providing impacted individuals with access to credit monitoring and identity protection services at no charge. To obtain more information about this incident or enroll in these services, individuals should contact A&PC’s dedicated assistance line at 1-800-405-6108, Monday through Friday, from between 8:00 AM and 8:00 PM ET, excluding holidays. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors. Individuals may also place a fraud alert or credit freeze by contacting the credit reporting agencies: TransUnion 1-800-680-7289; Experian 1-888-397-3742; Equifax 1-888-298-0045.
